Wellness Tip: Medical Checklist for Travelers


If you are someone who is under the care of a physician for a chronic illness, you should make sure that any time you are traveling, you complete the following checklist:


* Provide a copy of your itinerary to a family member or friend.
* Copy all medications and reasons for taking them and put them in your wallet.
* Keep an emergency contact phone number list in your wallet. Include family members, physicians, and pharmacy information.
* Provide a copy of your health insurance information to a family member or friend and keep your health insurance card in your wallet.
* Take a small basic first aid kit, especially if you will be going hiking or camping.
* Make sure you take your prescriptions in their original marked bottles.
* Check in with your friends or family a few times before coming home and notify them of any change in your itinerary.

· If you are diabetic, don’t forget:

* glucose meter
* extra lancets
* glucose strips
* alcohol pads
* batteries for your glucose meter
* insulin / medication
* MEDICAL ID BRACELET
* glucagon kit
* glucose tablets

· If you have asthma, don’t forget:

* inhalers
* medications
* MEDICAL ID BRACELET

· If you have severe allergies, don’t forget:

* Epi-pen
* Benadryl or generic
* MEDICAL ID BRACELET
